Les \u00e9chographies sont des outils cliniques cl\u00e9s utilis\u00e9s pour diagnostiquer de nombreuses pathologies, mais l'acc\u00e8s aux \u00e9chographies traditionnelles est limit\u00e9 par la disponibilit\u00e9 des machines et la capacit\u00e9 d'analyse des r\u00e9sultats. Les patients, en particulier ceux qui vivent dans des communaut\u00e9s rurales ou isol\u00e9es, peuvent \u00eatre confront\u00e9s \u00e0 de longs d\u00e9lais d'attente ou doivent parcourir de longues distances pour avoir acc\u00e8s \u00e0 ces \u00e9chographies.<\/p>\n\n\n\n

Providence Health Care est membre de DIGITAL, anciennement Canada's Digital Technology Supercluster, qui vise \u00e0 acc\u00e9l\u00e9rer les technologies num\u00e9riques canadiennes afin d'am\u00e9liorer la sant\u00e9 et le bien-\u00eatre, de prot\u00e9ger notre environnement et de stimuler la productivit\u00e9 \u00e9conomique. En collaboration avec PHC Ventures, Change Healthcare, Clarius Mobile Health, l'Universit\u00e9 de Colombie-Britannique et le Rural Coordination Centre of BC, et avec le soutien de la St. Paul's Foundation, le projet DIGITAL's Intelligent Network for Point-of-Care Ultrasound (IN-PoCUS) a d\u00e9velopp\u00e9, distribu\u00e9 et \u00e9tudi\u00e9 l'impact des ultrasons portatifs.<\/p>\n\n\n\n

Ces appareils combinent l'apprentissage automatique et une plateforme bas\u00e9e sur le cloud pour cr\u00e9er un r\u00e9seau d'\u00e9chographie sur le lieu de soins afin de fournir des diagnostics plus rapides et plus pr\u00e9cis et de permettre aux cliniciens de prendre des d\u00e9cisions plus \u00e9clair\u00e9es. \u201cL'un des avantages de la technologie des ultrasons est qu'elle peut \u00eatre utilis\u00e9e pour pratiquement tous les syst\u00e8mes organiques\u201d, explique le Dr Oron Frenkel, scientifique de Advancing Health et m\u00e9decin urgentiste \u00e0 l'h\u00f4pital Saint-Paul, qui a \u00e9t\u00e9 le chef de file clinique de ce projet de super-groupe. Avec les ultrasons portatifs, les m\u00e9decins peuvent scanner plusieurs parties du corps avec un seul appareil, au lieu de devoir passer par un processus plus long pour scanner une zone particuli\u00e8re avec l'imagerie traditionnelle. Cela peut contribuer \u00e0 r\u00e9duire les retards courants.<\/p>\n\n\n\n

Le Dr Frenkel n'est pas \u00e9tranger \u00e0 la technologie de l'\u00e9chographie portable. Il a suivi une formation en \u00e9chographie au centre m\u00e9dical du comt\u00e9 d'Alameda \u00e0 Oakland, en Californie, et a dirig\u00e9 pendant plusieurs ann\u00e9es son propre programme de formation \u00e0 l'\u00e9chographie par l'interm\u00e9diaire de Providence Health Care, en formant des m\u00e9decins urgentistes de toute la Colombie-Britannique, y compris ceux des zones rurales.<\/p>\n\n\n\n

Au cours du projet DIGITAL, le Dr Frenkel a pu utiliser les contacts qu'il avait \u00e9tablis gr\u00e2ce \u00e0 cette formation pour se mettre en relation avec le Rural Care Coordination Centre of BC, qui est devenu un partenaire essentiel.<\/p>\n\n\n\n

IN-PoCUS soutient les m\u00e9decins gr\u00e2ce \u00e0 une technologie d'intelligence artificielle int\u00e9gr\u00e9e \u00e0 l'appareil qui fournit un retour visuel et r\u00e9duit le besoin de formation sp\u00e9cialis\u00e9e, ainsi qu'\u00e0 un r\u00e9seau \u00e9ducatif solide qui leur permet d'acc\u00e9der aux conseils de sp\u00e9cialistes en cas de besoin. Le projet a permis de distribuer plus de quatre-vingts appareils d'\u00e9chographie portables \u00e0 des m\u00e9decins ruraux en Colombie-Britannique.<\/p>\n\n\n\n

Une \u00e9tude qualitative de ce programme, co-r\u00e9dig\u00e9e par le Dr Frenkel, a examin\u00e9 le point de vue des m\u00e9decins de famille ruraux sur IN-PoCUS et son impact sur les soins qu'ils prodiguent. L'\u00e9tude a montr\u00e9 qu'IN-PoCUS permettait aux m\u00e9decins de prendre de meilleures d\u00e9cisions et que ces m\u00e9decins avaient l'impression d'avoir plus d'outils \u00e0 leur disposition pour fournir de meilleurs soins. Les \u00e9chographes portatifs leur fournissaient un retour d'information pr\u00e9cieux en temps r\u00e9el, ce qui \u00e9tait particuli\u00e8rement important dans les communaut\u00e9s n'ayant pas acc\u00e8s \u00e0 des services d'\u00e9chographie formels. Les utilisateurs d'IN-PoCUS ont m\u00eame fait \u00e9tat d'une plus grande satisfaction au travail.<\/p>\n\n\n\n

\u201cL'\u00e9tude qualitative a vraiment mis en \u00e9vidence l'impact de l'acc\u00e8s \u00e0 cette technologie de diagnostic pour les m\u00e9decins ruraux\u201d, explique le Dr Frenkel.<\/p>\n\n\n\n

Une \u00e9tude quantitative portant sur la qualit\u00e9 de l'interpr\u00e9tation des images par les m\u00e9decins de famille par rapport \u00e0 celle des sp\u00e9cialistes est toujours en cours. Elle permettra d'identifier les domaines dans lesquels un meilleur soutien pourrait \u00eatre apport\u00e9.<\/p>\n\n\n\n

Un nouvel outil qui devrait devenir une norme de soins<\/strong><\/h2>\n\n\n\n

IN-PoCUS ne remplace pas les scanners et les sp\u00e9cialistes traditionnels, mais il constitue un compl\u00e9ment pr\u00e9cieux \u00e0 la bo\u00eete \u00e0 outils des m\u00e9decins. Le Dr Frenkel pense que ce n'est qu'une question de temps avant que les \u00e9chographies portables au point de service ne fassent partie de la norme de soins.<\/p>\n\n\n\n

Toute nouvelle technologie n\u00e9cessite un certain niveau de formation et d'\u00e9ducation pour apprendre \u00e0 l'utiliser de mani\u00e8re s\u00fbre et efficace. Au fur et \u00e0 mesure que les \u00e9chographes portatifs se r\u00e9pandent dans l'environnement de formation m\u00e9dicale et que de plus en plus de m\u00e9decins arrivent sur le terrain apr\u00e8s avoir \u00e9t\u00e9 form\u00e9s \u00e0 l'utilisation de ces appareils, ils seront int\u00e9gr\u00e9s de mani\u00e8re transparente dans leur pratique clinique.<\/p>\n\n\n\n
