Gr\u00e2ce \u00e0 sa r\u00e9putation internationale en mati\u00e8re de r\u00e9duction des risques, Providence Health Care a \u00e9t\u00e9 leur premier choix de partenaire clinique.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Le d\u00e9fi<\/strong><\/p>\n<p>Approximately eight per cent of people who are prescribed opioids for pain will develop an addiction to opioids. For many years, doctors didn&#8217;t have the information they needed to minimize this risk at the time of prescribing opioids, nor did they receive follow-up data to reduce the rates of addiction.<\/p>\n<p>&nbsp;<\/p>\n<p>&#8220;Ideally, people would not use opioids unless necessary. In addition to the risk of addiction, opioids have side effects like drowsiness, mental fog, nausea, constipation and slowed breathing,&#8221; says Dr. Ainsley Sutherland, Co-Physician Lead for the <a href=\"https:\/\/www.providencehealthcare.org\/en\/clinics\/transitional-pain-clinic\">Clinique transitoire de la douleur<\/a> at St. Paul&#8217;s Hospital.<\/p>\n<p>&nbsp;<\/p>\n<p><a href=\"https:\/\/www.digitalsupercluster.ca\/projects\/reducing-opioid-use-for-pain-management\/\" target=\"_blank\" rel=\"noopener\">NUM\u00c9RIQUE<\/a> and a consortium of industry and academia partners including CareTeam, Thrive Health, BC Children&#8217;s Hospital, Excelar and \u00a0Providence Health Care, invested in developing a clinical platform that could help doctors identify high risk patients before they had their surgery and support them after surgery to minimize the number who developed a dependence on opioids.<\/p>\n<p>&nbsp;<\/p>\n<p>&#8220;It is exciting to see an organization like Providence Health Care innovating to improve care,&#8221; says Mark Nazemi, Director, Clinical Solutions &amp; Innovation, Thrive Health. &#8220;They are willing to work with the tools and technologies we have available to find solutions and change how care is provided.&#8221;<\/p>\n<p>&nbsp;<\/p>\n<p><strong>La strat\u00e9gie<\/strong><\/p>\n<p>Le projet a \u00e9t\u00e9 lanc\u00e9 fin 2019. L'arriv\u00e9e de la pand\u00e9mie de COVID-19 au d\u00e9but de l'ann\u00e9e 2020 a mis le projet en veilleuse pendant plusieurs mois. L'\u00e9quipe de PHC Ventures a \u00e9t\u00e9 en mesure d'intervenir et d'assurer le leadership lorsque le projet a red\u00e9marr\u00e9 en 2021. Nous avons supervis\u00e9 de nombreux partenaires externes et g\u00e9r\u00e9 les processus internes, tels que les approbations en mati\u00e8re de confidentialit\u00e9 et d'\u00e9thique. Cela a permis \u00e0 nos experts cliniques, comme le Dr Sutherland, de consacrer leur temps et leur \u00e9nergie \u00e0 ce qui apporterait le plus de valeur ajout\u00e9e au projet.<\/p>\n<p>&nbsp;<\/p>\n<p>&#8220;One of the most useful outputs of this project was the risk assessment tool we developed with Thrive Health,&#8221; says Dr. Sutherland. &#8220;We integrated the tool into the standard health history questionnaire patients complete before surgery and we developed an algorithm to determine if a patient was high or low risk for prolonged opioid use after surgery.\u00a0 The longer a patient takes opioids, the greater the risk of developing an addiction.&#8221;<\/p>\n<p>&nbsp;<\/p>\n<p>Les patients \u00e0 haut risque ont \u00e9t\u00e9 orient\u00e9s vers la plateforme technologique Careteam, o\u00f9 ils ont b\u00e9n\u00e9fici\u00e9 d'un plus grand nombre d'interventions li\u00e9es \u00e0 la douleur afin d'am\u00e9liorer leurs r\u00e9sultats apr\u00e8s l'op\u00e9ration. Les patients \u00e0 faible risque sont rest\u00e9s sur la plateforme Thrive Health, o\u00f9 leur qualit\u00e9 de vie a \u00e9t\u00e9 surveill\u00e9e pendant un an apr\u00e8s l'op\u00e9ration. La plateforme Thrive Health a recueilli des donn\u00e9es sur les niveaux de douleur, la consommation de m\u00e9dicaments et les taux de r\u00e9admission. Elle a \u00e9galement propos\u00e9 aux patients des ressources \u00e9ducatives.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>L'impact<\/strong><\/p>\n<p>Ce projet a conduit \u00e0 la cr\u00e9ation de la Clinique transitoire de la douleur. Une fois la technologie mise en place pour identifier les patients \u00e0 haut risque, l'\u00e9quipe clinique s'est rendu compte qu'elle devait mettre en place des mesures de soutien suppl\u00e9mentaires pour aider ces patients. Avec le soutien de PHC Ventures, l'\u00e9quipe a pu obtenir le financement de la nouvelle clinique.<\/p>\n<p>&nbsp;<\/p>\n<p>En activit\u00e9 depuis 2021, la clinique est compos\u00e9e de deux m\u00e9decins, d'une infirmi\u00e8re navigatrice et d'un psychologue. L'\u00e9quipe rencontre les patients avant l'op\u00e9ration et les informe sur la mani\u00e8re de g\u00e9rer les risques de d\u00e9pendance aux opio\u00efdes. Apr\u00e8s l'op\u00e9ration, l'\u00e9quipe continue de voir les patients pour les aider \u00e0 g\u00e9rer la douleur persistante.
